Screening for an unhealthy body weight and developing follow up plan

Maintaining a healthy weight is important for your health. It decreases your risk for heart disease, stroke, diabetes, certain cancers and other health issues. Getting screened helps to determine if you are over- or underweight so that you and your physician can develop a plan for reaching your health goals.

About this measure

This measure tracks the percent of patients who had a follow-up plan created if their weight was not in the normal range.
