Mental Health Case Management with Ben Gordon Center

Mental health case management services include assessment, planning, coordination, support, and advocacy services for clients who need assistance with:

  • Gaining access to mental health, social, vocational, and educational services
  • Housing
  • Public income entitlements
  • Identifying and investigating available resources
  • Exploring options
  • Linkage with or referral to necessary resources
  • Other community services

Case management activities may include:

  • Identifying and investigating available resources
  • Building skills for daily living
  • Explaining options to the client
  • Client-centered consultation between the Providing staff member and other professional staff to improve client’s progress/outcome
  • Linking clients with necessary resources
