Overview

The Transitional Care Clinic includes a team of dedicated providers, social workers and health advocates who will work together to help meet your healthcare needs. You may receive Transitional Care following discharge from the hospital or Emergency Department. We are here to help ensure that you get the care and support you need.

Our clinic accepts all insurances that are accepted by NMG, and we will also accept patients with limited or no insurance.

Standard Parking

Up to 7 Hours: $13 7 to 24 Hours: $28

Parking Validation

Parking garage tickets must be validated each time a car is parked. Without validation, regular garage rates will apply. You can validate your ticket at the customer service desks on the first and second floors.
