Community Sponsorship or Donation Application

Please complete the form below to submit your application for funding. We consider two types of funding requests:

• Sponsorships are funds Northwestern Medicine may provide to an organization in exchange for promotional benefits, such as recognition or awareness.
• Donations are gifts that Northwestern Medicine may award to a charitable community group or program to address needs in the community.

Use the links at the bottom of this page to upload your 501(c) documentation and your completed W-9. You will get a confirmation email after you submit your application.

A committee will review your request. A member of our team will follow up with you within 30 days.
