What To Expect During Your Hospital At Home Stay

Hospital at Home Services
Our dedicated Hospital at Home team will work together to care for you. You will get in-person nurse visits and virtual physician visits. In addition, we will:
- Monitor your health in real-time.
- Perform scans and lab tests at your home or take you to the hospital for them.
- Provide you with your medications and meals, if you choose.
- Set up transportation between your home and the hospital when needed.
Enrollment
An enrollment nurse is available every day from 8 am to 6 pm. They can answer questions and talk with your loved ones.

Hospital at Home is currently available to eligible patients at Northwestern Medicine Catherine Gratz Griffin Lake Forest Hospital.

Costs
There is no additional cost to you when you enroll in Hospital at Home, as you are still an inpatient. This means you are admitted to the hospital and your insurance covers your care like a regular hospital stay.
Your Care Team
You have a full team caring for you, just like in the hospital:
- Your Northwestern Medicine physician and registered nurses
- Case managers who help plan and manage your care
- Therapists, including physical, occupational and speech therapists, if needed
- Other specialists

Meet the Team
You'll have a Northwestern Medicine team caring for you, just like in the hospital. Your physician will lead your team and meet with you in a daily virtual visit. Nurses will visit you in person twice a day to provide care. Case managers will help plan your care, and other specialists are available if you need, like physical, occupational and speech therapists.
