With Northwestern Medicine Complex Primary Care, you can receive consistent, holistic care to address multiple chronic medical conditions. Patients must meet eligibility requirements for Complex Primary Care, including:
- Patients must have Medicare or Medicare Advantage insurance.
- Patients must have a referral from their Northwestern Medicine primary care physician or a Northwestern Medicine specialist.
- Patients must meet specific medical risk conditions.
Parking and Transportation
