Our family medicine physicians offer diagnosis and treatment for everything from the common cold to chronic illnesses, in addition to routine screenings, annual check-ups, and other preventive medicine services to keep you and your family healthy. Care is provided by physicians who are part of Northwestern Medical Group and who serve on the medical staff of Northwestern Memorial Hospital and/or Northwestern Medicine Lake Forest Hospital.

There is a northbound entrance on Rockland Road (176) and southbound entrance on Shagbark Road. Exit off Rockland Road (176) for Highway 41 or the tollway.
