Preparing for Your Visit

If you’re transferring your care to Northwestern Medicine, copies of recent imaging and medical records will be required prior to scheduling your appointment. This is requested so that we may ensure your first appointment is as beneficial as possible. If you have not had any recent imaging studies within the last 12 months, please contact us for further information.

You may be asked to bring the following items:

• Imaging studies, laboratory studies and reports
• List of current medications
• Procedure reports relating to your condition and office notes from providers you may have seen previously

During your appointment, your doctor will review these materials with you and offer recommendations for treatment. For your convenience, a patient liaison will coordinate your entire care experience.

You will see a neurointerventionalist and if needed, a neurosurgeon or a neurologist on the same day.
