Long delay in completing the short-term disability paperwork needed for my salary to continue. Prescription for Plavix refill not sent to Lake Forest CVS pharmacy as indicated. I called Walgreens in Gurnee myself to get a refill. Wasn't notified that I needed a heart monitor applied at my follow-up appointment. However, medical knowledge and explanation was good.
