Care Coordinators Are Here to Help
Our care coordinators are your “care command center.” They are licensed medical professionals who work with your primary care physician or advanced practice provider to help you manage your health needs. They help ensure you have what you need from your healthcare team, including your doctor, social worker, pharmacist and community resource coordinator.
Care coordinators can:
- Update your primary care team on your care, medications and personal health information
- Co-create a plan of care for you to focus on your health needs and goals
- With your permission, review your medical history to identify and address your care needs or any concerns
- Connect you to community resources such as social services agencies and support groups
- Help you understand your medications and how to manage chronic health conditions
You and your care coordinator can work together over the phone or through messaging in MyNM.

Your Northwestern Medicine diabetes care team is here to help you balance life outside of the doctor's office. This is a team effort. They can connect you with resources, care and communities that will help you stay healthy. They can help you avoid more serious diabetes-related illness.
Each of our trained experts has a role to play to help you manage your diabetes.
Your physician
- Helps provide care by monitoring your diabetes through physical exams, lab tests and medication management
Your pharmacy team
- Helps with your medications and glucose monitoring
- Assists with coupons/vouchers so that you can save money with your medications
Your diabetes educator
- Educates and supports you in managing your diabetes using the 7 recognized self-care behaviors, which include:
- Healthy eating
- Being active
- Monitoring blood sugars
- Taking medication
- Problem-solving
- Reducing risks
- Healthy coping
Your nurse care coordinator
- Works with you to help you set health goals
- Answers your questions and checks in with you on a regular basis
- Provides you with knowledge and ability to understand and better manage your diabetes
Your social worker
- Offers support services to address both the emotional and social challenges of diabetes
- Connects you with community resources and organizations that help with things like:
- Insurance options
- Paying for care
- Housing
- Transportation
- Food
- Support groups
