About the Community Health Needs Assessment 

The Community Health Needs Assessment (CHNA) is used to identify prevalent health needs among residents across all socioeconomic groups, races and ethnicities. The CHNA also highlights health disparities which disproportionately impact medically underserved, low-income and uninsured populations. The information and insight gained through the CHNA will be used to guide new strategies and enhance existing efforts to improve the health of our communities. Northwestern Medicine and its community health partners share a vision of a healthy community, and we are committed to working together to address significant health needs and make the greatest possible impact on community health status.

For the time period of this report, fiscal year 2021 (FY2021), Northwestern Memorial HealthCare (NMHC) was the nonprofit corporate parent of Northwestern Memorial Hospital, Northwestern Medicine Lake Forest Hospital, Northwestern Medicine Central DuPage Hospital, Northwestern Medicine Delnor Hospital, Northwestern Medicine Kishwaukee Hospital, Northwestern Medicine Valley West Hospital, Northwestern Medicine McHenry Hospital, Northwestern Medicine Huntley Hospital, Northwestern Medicine Woodstock Hospital, and Marianjoy Rehabilitation Hospital, part of Northwestern Medicine. NMHC was also the nonprofit corporate parent of Northwestern Medical Group, Northwestern Medicine Regional Medical Group, Northwestern Memorial Foundation and other subsidiaries. As of January 1, 2021, NMHC is also the nonprofit corporate parent of Northwestern Medicine Palos Hospital and Northwestern Medicine Palos Medical Group; Palos Hospital and Palos Medical Group are included in this report for the period of January 1, 2021, to August 31, 2021. On September 1, 2021 — beyond the scope of this report — the operations of Palos Medical Group were merged into Regional Medical Group.

NMHC submitted the Annual Nonprofit Hospital Community Benefits Plan Report for FY2021 to the Illinois Attorney General on behalf of each of its subsidiaries.
