MyNM Proxy Access

Proxy access gives someone else access to a patient’s MyNM record. The person with proxy access can:

• View the patient’s information, including medications, health history, test results, clinical notes, discharge instructions and After Visit Summaries
• Request medication refills
• Request, schedule and manage appointments
• Send secure messages to the patient’s care team

If you are a MyNM user, you can invite someone to be your MyNM proxy electronically using the Share My Record function within MyNM.

Otherwise, please complete the appropriate proxy form to submit your request.
