Through the Northwestern Medicine Concierge Cardiology program, patients pay an annual membership fee to receive comprehensive cardiovascular care from a dedicated physician.
As a patient in this program, you receive expert heart care and personalized recommendations that support your heart health and overall well-being. You also receive direct access to your physician and care team whenever you need it.
We currently only offer concierge cardiology services in Lake Forest, IL.
While you may enroll in both the Concierge Cardiology program and the Concierge Primary Care program, there are separate membership fees for each.

Who Can Enroll in the Concierge Cardiology Program
While anyone who is interested in maintaining long-term cardiac wellness can enroll in this program, it is ideal for:
Patients who have had a cardiac event or heart disease diagnosis and want to maintain cardiac wellness.
This includes people who have experienced:
- Heart attack
- Stent placement
- Bypass surgery
- Arrhythmia
- Heart failure
Patients who are concerned about their risk for heart disease and other vascular conditions.
This includes:
- People with a family history of heart or vascular disease
- People who have never had a cardiac event, but want to invest in wellness
- People who have other cardiac risk factors they want to control
- People who want to stay up-to-date with the latest medical literature and clinical trials related to heart health
- People interested in getting advanced lab work and cardiac testing
What to Expect During a Concierge Cardiology Visit
You will see your doctor at a minimum of every six months for a 45-to 120-minute visit. During these visits you may:
- Address any medical issues or symptoms you’ve been experiencing
- Receive heart health education and wellness counseling
- Talk about your diet and lifestyle
- Discuss the latest clinical guidelines and prevention strategies for improving your heart health
You may also receive routine bloodwork during your visit. We conveniently offer electrocardiograms (EKGs) and Holter monitor placement at our office.

We can also support you with scheduling visits on request. If you need advanced care, such as cardiac surgery, or stent or pacemaker placement, your care team will help you navigate this every step of the way. Cardiac diseases typically impact multiple organ systems, not just the heart. Your physician will communicate with any other specialists caring for you to make sure you understand your treatment plan.

The Concierge Cardiology program at Northwestern Medicine gives you dynamic access points to your doctor and care team, minimizing stress and giving you enough time to ask questions, get answers and plan your care. Your doctor, nurse and coordinator are your main points of contact. It’s easy and quick to connect with them whenever you need to.

About Ian D. Cohen, MD
Dr. Cohen is an interventional cardiologist with more than 30 years of experience. He’s spent decades performing complex cardiac procedures and caring for all types of heart and vascular conditions. This breadth of work allows him to see cardiac issues holistically and to anticipate patients’ needs. He recognizes that cardiac conditions are chronic illnesses that need more specialized, continuous attention. Dr. Cohen prioritizes listening and building personal relationships with his patients. He is chief of Cardiology for the north and northwest regions of Northwestern Medicine where he manages more than 20 world-class cardiologists. He also serves as faculty at Northwestern University Feinberg School of Medicine.
