Through our Concierge Primary Care program, patients pay an annual membership fee to receive direct, quick access to a highly attentive primary care doctor. Our physicians work to thoroughly understand your lifestyle and health history to help guide you through every step of your health and wellness journey.

While you may enroll in both the Concierge Cardiology program and the Concierge Primary Care program, there are separate membership fees for each.

Who Can Enroll in the Concierge Primary Care Program

Patients of all ages can enroll in this program, but we currently offer pediatric concierge medicine only in the western suburbs. This program is ideal for:

  • People who want to prioritize preventive health and wellness
  • People with busy schedules
  • People who travel frequently and need reliable access to their doctor while traveling
  • People who value consistency and care about who is taking care of them
  • People who are looking for an easier way to communicate with their care team
  • People with chronic diseases

What to Expect During a Concierge Primary Care Visit

You will see your doctor at a minimum of every 12 months for a 45-to-120-minute visit in a comfortable, welcoming office environment.

During these visits you may:

  • Address any medical issues or symptoms you’ve been experiencing
  • Receive health education and wellness counseling
  • Talk about your diet and lifestyle. Your primary care doctor can also arrange separate consultations with a dietitian or exercise physiologist.
  • Discuss the latest clinical guidelines and prevention strategies for improving your health

People with chronic diseases will have more frequent scheduled touchpoints with their doctor for disease management and medication monitoring. For example, our doctors see patients with diabetes at least four times per year and patients with high blood pressure at least twice per year.

Outside of annual checkups, it is quick and easy to schedule a sick visit with your doctor, with appointments available on a same- or next-day basis.

We offer free parking for all concierge medicine patients at all locations.

Your physician and their dedicated staff coordinate your care across the Northwestern Medicine health system, including communicating with other physicians you see. We can also support you with scheduling visits on request. This is why concierge primary care is beneficial for people with chronic conditions. Care coordination goes beyond referrals. If you see another specialist, your concierge primary care doctor will make sure you understand your treatment plan and get the follow-ups you need.
