Medicaid Transition

Starting January 1, 2018, the Illinois Department of Health Care and Family Services (HFS) will expand Medicaid managed care to include all counties across Illinois. The new expanded program is called HealthChoice Illinois. While this change will impact your Medicaid managed care plan choices in the state of Illinois, your Medicaid benefits will remain the same.

There are several Medicaid managed care plans that will not be participating in HealthChoice Illinois, including Aetna Better Health, Family Health Network and Community Care Alliance of Illinois. If you are currently enrolled in one of these plan, you will need to enroll in a new plan that includes Northwestern Medicine to continue to access care with a Northwestern Medicine provider or at one of our facilities.  

To assist you during this transition, all NM patients enrolled in one of the Medicaid managed care plans that will be discontinued in 2018 recently received a Medicaid Managed Care Plan Transition letter (English | Spanish).

As of January 1, 2018, Northwestern Medicine will continue to accept the following Medicaid managed care plans:

  • BlueCross Community Options Health Plan
  • CountyCare (Cook county residents only)
  • Meridian Health Plan

If you were recently notified by HFS or a managed care plan provider that you were auto enrolled in one of the three (3) plans listed above, and want to stay in the plan that was selected for you, you do not need to make any changes at this time.

If you wish to select a new Medicaid managed care plan or change the plan that was selected for you, you may do so within 90 days from January 1, 2018.
