Meridian Medicaid Termination

Meridian Health Plan of Illinois ended its Medicaid managed care contract with Northwestern Medicine on December 31, 2024. Starting January 1, 2025, patients with this health plan had to choose a new plan to continue to get care at Northwestern Medicine.
We sent letters and emails to patients about the change. Meridian also sent information to patients. Some people qualified to continue care at Northwestern Medicine until March 31, 2025. We now accept these Medicaid managed care plans:
- Blue Cross Community Health Plans
- CountyCare (Cook County residents only)
- Molina Healthcare of Illinois

When does my continuity of care end at Northwestern Medicine?
Continuity of care for qualifying patients with Meridian Medicaid insurance expired March 31, 2025. If you were in this group, our Financial Counseling team would have contacted you.
Continuity-of-care exceptions will continue for those who have a critical clinical need for ongoing services. This could include those close to delivering a child or undergoing cancer treatment.
