Heart Failure Program

The Community-Based Heart Failure program helps heart failure patients and their families learn how to manage their heart failure at home. The program will provide heart failure education, resources, and support to help the patient to be successful in self-management of their heart failure.

The program helps you and your family:

  • Learn about heart failure and understand your condition
  • Know what medications you are taking and how they work
  • Recognize symptoms that may indicate that your condition is worsening and may need medical attention

A community nurse will work with you to help you monitor your daily weight and blood pressure, assess how you are feeling, and discuss any physical symptoms you may be having. In addition, a dietitian helps address your nutrition needs and answers any questions you may have.
