Northwestern Medicine Labor and Delivery Pre-registration
The MyNM Patient Portal, allows you to pre-register for your upcoming delivery with us. Additionally, the MyNM Patient Portal allows you to manage your care online or via the MyNM® app through a variety of tools after receiving care at one of our NM inpatient or outpatient locations.
Once logged in to the MyNM Patient Portal, you will be automatically directed to the "Register for Your Delivery" under the Visits tab.
Through a series of steps, you will be asked to provide detailed information needed to get you pre-registered in anticipation of your upcoming delivery at NM. If you are already an existing patient with NM, some of this information may be pre-populated for you. You will simply need to validate that the information we have is up to date.
- Physician name:
- Physician/midwife: It is important that we know the name of your obstetrician or nurse-midwife. If you have chosen a midwife for your delivery, please provide the name of the attending physician who is associated with the midwife.
- Pediatrician: Before your baby is born, you should choose a pediatrician or other healthcare provider to care for your baby after birth. If this provider is on the NM staff, they or someone from their practice will care for your baby while in the hospital. If not, or you have not chosen a pediatrician by the time you deliver, a hospital-based provider will be assigned to care for your baby while in the hospital.
- If you have not chosen a pediatrician and would like an NM provider, you can contact our Physician Referral Department at 844.344.6663, TTY 711, for assistance.
- Guarantor name, address, and phone number: This is the person responsible for the bill.
- Insurance company and identification numbers: You will be able to enter a primary and a secondary, but can also indicate if you don't have insurance.
- Emergency contact
If you feel you cannot complete your pre-registration within 30 minutes, please exit now, gather your information, and return when you are ready.
