For Referring Physicians

Thank you for considering a referral to the Northwestern Medicine Neurobehavior and Memory Clinic. We appreciate the opportunity to provide consultation services to you and your patients.

Our clinical services are focused on providing a diagnostic evaluation and treatment recommendations for neurodegenerative diseases, particularly for patients who are experiencing early or unusual changes in memory, behavior, language, movement, visual perception, or other cognitive functions. This includes diagnoses such as Alzheimer’s disease, primary progressive aphasia, frontotemporal dementia, corticobasal degeneration and progressive supranuclear palsy, posterior cortical atrophy, vascular dementia, Creutzfeldt-Jakob disease, and dementia with Lewy bodies.

We look forward to collaborating with you on your patient’s treatment plan. To make a referral, please include the following information in a referral letter: 1) Information about the patient, including name, date of birth, telephone numbers, address, and name of the patient’s primary care physician, if known; 2) Information about yourself, including your name, specialty, telephone numbers, and fax number; and 3) Reason for consultation.

Please also provide the following records to facilitate review:

  • Medical Notes from neurologists, psychiatrists or family physicians
  • Previous neuropsychological testing reports
  • Laboratory reports such as:
    • EEG
    • Blood tests
    • Sleep study
    • MRI on CD of the brain
    • CT on CD of the brain
    • PET on CD of the brain
    • SPECT on CD of the brain
  • Current list of all medications including vitamins or aspirin.

Records that are 20 pages or less may be faxed to 312.695.6072.

Once received, all referrals are carefully reviewed by our team to determine the most appropriate evaluation. The review process takes two (2) business days upon receipt of records. If a patient requires more urgent attention, please call 312.695.9627 to speak with one of our clinicians.
